POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – NC/compact RN license; BLS; 2 years recent acute hospital case management experience

LENGTH OF ASSIGNMENT – 26 weeks

SHIFT / HOURS PER WEEK – 8:00AM-4:30PM

SYSTEMS – CERNER, NATE, ENSOCARE

START DATE – 7/7/2025
