Bristol Hospice in Appleton, WI is seeking an RN Case Manager to coordinate end‑of‑life care and support patients and families in a compassionate, patient‑centered environment.

You will develop care plans, monitor health status, and collaborate with a multidisciplinary team to ensure comfort, dignity, and quality of care. Full‑time, Monday–Friday, 8am–5pm, with a salary range of $88,000–$97,000 per year.

Qualifications

• Valid RN license in good standing in the state of employment.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Strong observational and communication skills—written and verbal.
• Flexible, team‑oriented attitude and willingness to support additional duties.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support to patients and families, ensuring comfort and dignity.
• Assess physical, emotional, and spiritual needs throughout care journey.
• Monitor and document vital signs accurately.
• Administer physician‑ordered medications and treatments.

• Keep detailed and up‑to‑date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Contribute to other supportive duties as part of a compassionate, mission‑driven team.
