The position requires graduation from an accredited nursing program, SC-licensed RN (compact state acceptable), BLS, and 1 year of case management experience; BSN preferred and case management

Qualifications

• Graduate from an accredited school of nursing.
• Registered Nurse license required.
• Case Management certification preferred.
• Basic Life Support (BLS) required.

Responsibilities

• Review patient records to create a care plan that helps patients achieve their goals.
• Develop, document, and implement plans providing resources to address social, physical, mental, emotional, spiritual, and supportive needs.
• Review admission criteria to assess appropriateness of admission, continued stay, and level of care.
• Assist patients, families, and physicians with post-acute placement.
• Advocate for patients ensuring cost-effective, high-quality care.
• Refer cases not meeting criteria to the designated Physician.
• Educate patients and families about treatment options and health status.
• Perform additional assignments from Case Management Department Head.
