Elevance Health is seeking a Manager I Case Management to lead a team of nurse care managers. This role focuses on assessing, developing, implementing, coordinating, monitoring, and evaluating care plans to optimize member health care across the care continuum and ensure access to services aligned with health needs.

This hybrid position requires in-office presence 3 days per week, with flexibility for remote work as appropriate.

Qualifications

• Requires BA/BS in a health-related field and minimum of 5 years of clinical experience.
• Current, unrestricted RN license in the applicable state(s) is required.

Responsibilities

• Lead process improvement, quality assurance, and adherence to medical policy and care management standards.
• Hire, train, coach, counsel, and evaluate the performance of direct reports.
• Maintain certification requirements and licensure where applicable for managers of case management processes.
