Job summary

Elevance Health is seeking a Nurse Case Manager I to manage care for members with complex needs. The role is virtual full-time with required in-person training, and occasional office visits within commuting distance.

Work hours are 8:00–16:30 with two shifts per month 11:30–20:00. Responsibilities include assessing needs, developing and implementing care plans, coordinating care with providers and payers, and monitoring outcomes.

Qualifications

• BA/BS in a health-related field required.
• Current, unrestricted RN license in applicable state(s).
• Multi-state licensure if providing services in multiple states.

Responsibilities

• Perform care management telephonically or on-site for members with complex and chronic needs.
• Assess, develop, implement, coordinate, monitor and evaluate care plans to optimize member health.
• Coordinate authorizations/referrals within benefits or extra-contractual arrangements.
• Interface with Medical Directors and Physician Advisors on care plans.
• Negotiate reimbursement rates as applicable.
