Job summary

Diversity Group is seeking a Registered Nurse - Case Management to coordinate patient care, discharge planning, and education across inpatient and outpatient settings. The CM RN will collaborate with a wide interdisciplinary team to ensure timely, safe outcomes for Veterans and their families.

The role requires active RN licensure in the United States, English proficiency, and experience in case management, utilization review, or discharge planning. VA benefits and structured rewards apply.

Qualifications

• Graduate of a school of professional nursing approved by accrediting agencies (ACEN/CCNE)
• State license - active and unrestricted
• English language proficiency for direct patient care
• Eligibility for VA nursing standards and education equivalency as needed

Responsibilities

• Provide and supervise nursing care, assess plans and evaluate outcomes
• Coordinate care focused on patient education, self-management, and satisfaction
• Collaborate with interdisciplinary teams to improve care and discharge planning
• Lead in policy implementation, evaluation and guidelines updates
• Support peers and mentors in CM knowledge sharing and professional development
