• Coordinates care with multiple providers across all levels and sites of care
• Assesses and documents CM outcomes and participates in development and revision of policies and guidelines
• Supports peers with CM knowledge sharing to provide safe, high-quality nursing care
• Ensures timely follow-up and assists patients and families with access to VA services
