Responsibilities

• The RN-Case Manager is responsible for assessing, planning, implementing, and evaluating patient care while ensuring age-specific needs are met.
• Coordinates care, patient education, and self-management; collaborates with interdisciplinary teams; follows established policies; demonstrates leadership and creative problem-solving in managing complex patient needs.
• Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
• Addresses psychosocial, nursing and medical needs of patients and their families/caregivers through interdisciplinary patient care management practice.
