Job summary

The Case Management (CM) Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients. The CM RN coordinates care with multiple providers across all levels and sites of care and leads discharge planning for inpatient and outpatient settings.

This role emphasizes collaboration with veterans, families, and caregivers to meet complex health care needs, and includes involvement in education, competency development, and outcomes evaluation.

Qualifications

• English language proficiency required.
• State license as Registered Nurse in any US state or territory or DC.
• Graduate of a school of nursing approved and accredited by ACEN or CCNE.
• Bridge program options for master’s level candidates may be considered.
• Temporary appointment possible before licensure; conversion to permanent on graduation.
• Foreign nurse graduates may use degree equivalency evaluation for licensure credentialing.

Responsibilities

• Provide competent, evidence-based care to assigned patients.
• Coordinate care, discharge planning, and patient/family education.
• Collaborate with interdisciplinary team to ensure safe transitions of care.
• Direct nursing education, orientation, and competencies; support quality improvement.
• Lead and coordinate care across inpatient and outpatient settings.
