Job summary

Molina Healthcare in Illinois seeks a Transition of Care Coach (RN) to support care transition activities from hospital admission through post-acute settings. You will help ensure timely discharge planning, coordinate with hospital staff, providers, and families, and reduce readmissions by guiding patients through the 30-day transition period.

The role requires an active RN license, strong communication, and the ability to travel locally.

Qualifications

• Active RN license in state of practice.
• Minimum 2 years healthcare experience with 1 year hospital discharge planning, care management or behavioral health experience.
• Valid driver's license and reliable transportation.
• Knowledge of Care Transitions Intervention (CTI) or similar model.
• Background in discharge planning and/or home health.
• Ability to work independently and handle high-pressure situations.

Responsibilities

• Follows member through a 30 day program from hospital admission to post-acute settings to reduce readmissions.
• Collaborates with discharge planners, hospitalists, providers, facility staff and family.
• Ensures safe transitions with adequate caregiving and medical oversight.
• Coordinates with ICT and care teams for seamless care delivery.
• Educates member on seven Transition of Care pillars and uses motivational interviewing.
• Travel 40-50% as needed.
