Job summary

Qualifications

• RN licensure in Massachusetts.
• 3–5 years in an acute care setting.
• ACM, CCM, or CMAC preferred.

Responsibilities

• Collaborates with the health care team to develop the plan of care and patient flow.
• Reviews cases within 24–48 hours to assess LOS and discharge planning.
• Coordinates transition plan with social workers, direct care providers, and post-acute providers.
• Communicates the plan to patient, family, and interdisciplinary team.
• Documents care plans and ensures alignment with department policies.
