Qualifications

• Associate's Degree in Nursing required.
• RN license in Indiana required.
• BLS certification required.
• 5 years nursing/patient care experience required.
• 1 year case management or care coordination experience preferred.

Responsibilities

• Coordinate care transitions between levels of care with care coordination.
• Act as liaison between patients, physicians, and outside agencies to coordinate appointments and admissions.
• Coordinate exchange of information before and after patient visits.
• Collaborate with the healthcare team to coordinate patient care.
• Meet patients and families to complete psychosocial history and assess family dynamics.
• Review patient records and history to assess current needs.
• Assess Plan of Care to meet patient needs.
• Provide education for families, patients, and healthcare team.
