Qualifications

• Current active multistate RN license in good standing in the state of residence.
• 2+ years of RN experience.
• BLS/BCLS certification within 14 days.
• High school diploma and BSN as required education.
• Basic typing and word processing ability.
• Excellent analytical, critical-thinking, and problem-solving skills.
• Previous medical office experience preferred.

Responsibilities

• Manage patient flow, triage, and direct patient care.
• Coordinate communication among front desk staff, nursing staff, physicians, and the clinic manager.
• Develop expected patient outcomes, define resource utilization, and establish patient goals with providers.
• Provide nursing support and care coordination through clinical and educational programs and support services.
• Support patient self-management of disease.
• Coordinate continuity of care after hospital admissions, discharges, and emergency room visits.
• Manage high-risk patients using a patient registry.
• Assess patients' physical, psychosocial, financial, and environmental healthcare needs.
• Educate patients and staff on healthcare needs, safety, medications, diagnostics, nutrition, and wellness.
• Document assessments, treatment plans, clinical findings, and progress in the EMR.
• Observe, record, and report patient conditions and reactions to drugs and treatments.
• Maintain and review patient records and follow up on tests and results.
• Respond to and refer patient treatment-related phone calls.
• Maintain patient confidentiality.
