Job summary

Jobtailor is seeking a proactive healthcare liaison to build strong relationships with referral sources and increase patient starts. You will be the primary contact to educate referrals, patients, and families about Option Care services and coordinate care transitions.

You will collaborate with account leadership to develop and execute growth plans, coordinate with hospitals and facilities, and ensure smooth discharge processes to home or ATS.

Qualifications

• Licensed RN or Pharmacist in the state of practice with at least 2 years in healthcare.
• Experience building relationships with individuals at all organizational levels and with vendors.
• Knowledge of ACHC, URAC standards and related healthcare compliance.
• Experience delivering customer service to internal and external customers and measuring satisfaction.
• Proficient PC skills including internet, email, MS Word/Excel/PowerPoint.
• Ability to identify operational issues and implement process improvements.
• Must have reliable transportation with travel to facilities, home visits, and hospitals.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Serves as key contact to educate, assist, and service referral sources.
• Interacts with area providers to promote Option Care services for discharged patients.
• Assesses hospital patients to determine eligibility for alternative site care.
• Educates patients and families about services and promotes care transition.
• Coordinates discharge plans to home or ATS with care teams.
• Collects pre-admission information and secures signoff.
• Collaborates with Account Manager and Regional Sales Director on growth plans.
• Communicates progress and feedback to opportunities for promoting services.
• Delivers education programs to patient/caregiver and referral sources.
• Reviews medical records to gather history and insurance information.
• Initiates care transition coordination with referral sources and partners.
• Participates in data collection for therapy start and patient tracking.
• Maintains patient confidentiality and safeguarding information.
• Provides input on patient assessment and transition to home care.
• Acts as point of contact with other providers and coordinates needs.
• Arranges for special medical supplies or appliances as required.
