Bethesda Health Group in Belleville, IL seeks a Registered Nurse to assess residents, craft care plans, and coordinate with an interdisciplinary team across admissions and ongoing care. You will document plans, perform nursing interventions, and communicate with families, while maintaining patient safety and regulatory compliance.

The role requires critical thinking, strong communication, and the ability to manage time and mobility for demanding shifts.

Qualifications

• Education from an accredited school of nursing resulting in RN license.
• Current RN license in the state you will be working in.
• CPR/AED certification required, or willing to obtain within six months of employment.
• Long-term care experience preferred.

Responsibilities

• Assess residents' health status on admission and on an ongoing basis.
• Develop dynamic care plans with the interdisciplinary team (IDT).
• Review and revise care plans as needs change. Communicate plans to IDT, residents, and families.
• Document care plans on admission, cognition changes, or regulatory requirements.
• Identify nursing interventions including medications, treatments, and IV fluids; perform interventions safely and timely.
• Notify the physician using SBARR when changes occur.
• Assist with call lights and resident care (ADLs).
• Uphold current nursing practices and continue self-study.
