Job summary

INTEGRIS Health in Oklahoma City, OK is seeking a Day shift RN Care Coordinator for Population Health to support transitions of care and care coordination across disciplines.

The role focuses on assessing patient needs, coordinating with the care team, and educating patients and families to improve outcomes. Strong communication, organization, and driving comfort are essential in this position.

Qualifications

• Current RN license in Oklahoma or multi-state via compact license (eNLC).
• Bachelor's degree in nursing preferred.
• Minimum 3 years clinical experience in diverse settings.
• BLS within 90 days of hire and renew per guidelines.
• Care coordination experience preferred.
• Excellent interpersonal and collaboration skills.
• Computer experience required; Windows and Excel preferred.
• Must have valid Oklahoma driver's license and acceptable driving record.

Responsibilities

• Assess patients for clinical, psychosocial, financial and other factors affecting health outcomes.
• Evaluate social determinants of health and their impact on risk and outcomes.
• Collaborate with care team to provide high quality, patient-centered care.
• Provide patient education.
• Coordinate with patients/families/caregivers in goal setting and plan of care.
• Arrange services among community resources and providers.
• Identify barriers to reaching goals and develop strategies to overcome them.
• Promote wellness and preventative care education.
• Conduct Transitional Care Management per standard work.
