Job summary

St. Elizabeth Healthcare is seeking a Registered Nurse for a Care Coordinator role in the Fort Thomas, Kentucky hospital. The position supports the Care Coordination unit, focusing on efficient patient flow and coordinated transitions across care settings.

You will collaborate with patients, families, social workers, physicians and the care team to optimize outcomes, length of stay, and satisfaction. Shift is 8:00 am to 4:30 pm, 16 hours weekly, Sundays and Mondays.

Qualifications

• Bachelor’s degree in nursing from an accredited school; eligible for compact multi-state license.
• Licensed as an RN in the state of work; multi-state license accepted.
• Meets contact hour requirements for licensure.

Responsibilities

• Identifying, initiating and managing optimum patient flow/throughput to enhance continuity of care.
• Planning and facilitating coordinated, safe transitions to the next level of care required.
• Promoting patient satisfaction and quality outcomes.
• Targeting an optimal length of stay based on patient response to treatment and procedures.
• Providing care management planning to ensure quality patient care and regulatory compliance.
