Job summary

The University of Kentucky is seeking an RN Case Manager to facilitate appropriate care coordination for patients and their families. The role involves organizing and facilitating access to tests, procedures, and results to prepare discharge and assisting in the formation of discharge plans aligned with clinical needs.

You will serve as a liaison with other departments and agencies to ensure continuity of transition of care, arrange home health, DME, or infusion therapy, and complete referrals.

Qualifications

• RN licensure required.
• Experience in case management or discharge planning preferred.
• Strong communication and active listening skills.
• Proficiency with Microsoft Office and clinical documentation.

Responsibilities

• Coordinate care to prepare discharge plans.
• Facilitate access to tests, procedures and results.
• Liaise with departments and agencies to ensure continuity of care.
• Arrange home health, DME, or infusion therapy referrals.
• Provide training on case management services.
• Collaborate with SW-Case Manager and support high-risk referrals.
