Qualifications

• BSN from an accredited school.
• Current RN license valid for the work state.
• BLS certification preferred and required within 3 months of hire.
• OCN certification required within five years of hire.
• Ambulatory coordination experience preferred.

Responsibilities

• Provides high-quality nursing care to patients across the care continuum.
• Performs initial and ongoing patient assessments and develops care plans.
• Collaborates with physicians and multidisciplinary teams.
• Delivers patient and family education about diagnoses, treatments and follow-up.
• Maintains documentation in the hospital information system and adheres to protocols.
