Job summary

Southcoast Health is seeking an RN-Care Navigator for the OB/GYN service line in New Bedford, MA. The role supports care coordination across the continuum and involves education and resource provision for patients and families.

Day shift, 8:00am - 4:30pm, 24 flex 32hrs with location at OBGYN – New Bedford. Minimum qualifications include BSN and MA/RI RN licensure, 4 years of clinical experience (2+ in related setting), and BLS certification.

Qualifications

• Graduate of an accredited School of Nursing; BSN preferred.
• Massachusetts RN licensure required (MA/RI).
• 4 years of clinical experience with at least 2 years in a related setting.
• BLS certification required.
• Oncology and/or GYN experience preferred.

Responsibilities

• Provide consistent care coordination across the continuum for assigned patient population.
• Organize and facilitate clinical visit needs and education for patients and families.
• Support patients and families with information and resources for available services.
• Create, monitor, and improve clinical quality indicators for program evaluation and regulatory requirements.
• Facilitate case conferences and document outcomes with treatment recommendations.
