Job summary

Christian Care Ministry seeks an experienced RN Care Manager to coordinate care for members during acute, chronic, or costly health events. You will guide providers and members through Medi-Share processes and ensure eligible plans are followed.

Responsibilities include educating teams, verifying plan eligibility, and coordinating with multidisciplinary colleagues to achieve cost-effective, quality care in a faith-centered setting.

Qualifications

• RN with CM experience or case management exposure preferred.
• Proficiency in HIPAA guidelines and healthcare records handling.
• Experience coordinating care across providers and systems.

Responsibilities

• Acts as primary care coordinator for members in acute/chronic health events.
• Educates members/providers on Medi-Share provisions and processes.
• Verifies eligibility of care plans and directs appropriate resources.
