Job summary

Christian Care Ministry is seeking an RN Care Manager to serve as the primary care coordination contact for Medi-Share members in Florida. You will navigate the Medi-Share provisions with members and providers, coordinate care, document activities, and uphold HIPAA standards while delivering compassionate, biblically grounded service.

Qualifications

• Registered nurse with current unrestricted multi-state compact license.
• 3+ years nursing experience required, acute and catastrophic care highly preferred.
• 2+ years of case management and/or utilization management required.
• Bachelor of Science in Nursing preferred.

Responsibilities

• Coordinate care for members with acute, catastrophic, or costly health events.
• Educate members and providers on Medi-Share provisions and processes.
• Confirm member’s condition and plan of care eligibility for sharing per Medi-Share guidelines.
• Identify opportunities for member participation in programs like patient assistance for meds and tests.
• Provide clinical education and use coaching skills to improve outcomes.
• Maintain and document caseloads and workflows within service levels.
• Collaborate with interdisciplinary team and providers while ensuring HIPAA compliance.
