Petaluma Health Center is hiring a Case Manager Registered Nurse to coordinate the care of high-risk, complex patients across transitions from acute care to home and community resources. You will work with patients, families, and a multidisciplinary team to create individualized plans and ensure safe medication use.

Required: California RN license, driver’s license, BLS; ASN/ADN with BSN preferred. This on-site role offers robust benefits and opportunities for impact in geriatric and vulnerable

Qualifications

• RN licensure in California is required.
• Active California RN license in good standing required.
• BSN preferred for enhanced care management.
• Driver’s license in good standing required.
• BLS certification required.

Responsibilities

• Coordinate care for high-risk, complex patients transitioning from acute care to home or community resources.
• Collaborate with patients, families and interdisciplinary teams to develop individualized care plans.
• Assess medications, provide education on adherence and coordinate with pharmacy services.
• Review medications and adjust per protocol with provider support.
• Document care plans and outcomes in the electronic medical record.
• Conduct phone triage assessments and advise adult/geriatric patients per PHC protocols.
• Support quality improvement initiatives to reduce readmissions and improve access.
