Job summary
AccentCare is seeking an experienced RN Case Manager for Home Health in Petaluma. The role supports patients across American Canyon, Napa, Rutherford and Calistoga with in-home care, coordinating visits and services Monday through Friday from 8 am to 5 pm.
The candidate will be a licensed RN with CPR certification, willing to travel to multiple locations, and able to operate in a compassionate, patient-focused team.
Qualifications
- Licensed to practice as a registered nurse in the state of agency operation.
- Must possess and maintain valid CPR certification while employed in a clinical role.
- Must be a licensed driver with an automobile that is insured and able to travel to all business locations.
