Qualifications

• Graduate from an accredited school of Professional Nursing.

Responsibilities

• Collaborate with a multidisciplinary care team to coordinate care for high-risk patients across the continuum.
• Conduct comprehensive assessments addressing disease, age-specific, medical and psychosocial needs.
• Provide education, coaching and care plan updates to patients and families.
• Assist in transitions of care and ensure optimal communication among care settings.
• Identify barriers and connect patients to community resources.
