Job summary

Cleveland Clinic Weston Hospital is seeking an RN Care Coordinator to lead longitudinal care coordination for GI/IBD patients, overseeing tests, procedures, imaging, and results to guide treatment decisions. You will partner with a multidisciplinary team to optimize care delivery and patient outcomes.

The role emphasizes patient education, adherence monitoring, and coordination of transitions across care settings, with a day schedule from 8:00 a.m. to 5:00 p.m. in a hospital setting.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification through the American Heart Association or American Red Cross.
• Three to five years of nursing experience.

Responsibilities

• Coordinate and navigate GI/IBD patients through all phases of treatment.
• Triages patient concerns to support timely resolution.
• Collaborate with multidisciplinary care teams across the continuum of care.
• Educate patients and families on care plans and self-management.
• Identify barriers and connect to resources to improve outcomes.
