Job description

The MIGS Patient Navigator is an independent registered nurse who coordinates and facilitates comprehensive care for patients within the Minimally Invasive Gynecologic Surgery division. This role ensures seamless patient experiences by managing referrals, scheduling, pre‑ and post‑operative education, and follow‑up care while serving as a liaison between patients, providers, and referring physicians. The navigator enhances access to specialized services, supports informed decision‑making, and promotes continuity of care by serving as a direct liaison and primary point of contact for referring providers and specialty groups, ensuring seamless communication, coordinated services, and efficient patient transitions.

This position supports providers and patients at all office locations across the practice plan and requires travel to all clinical sites as well as travel to referring community offices as necessary.

Care Coordination, Patient Navigation & Pre‑Operative/Post‑Operative Education - 50%

Serves as the primary liaison and patient navigator, ensuring seamless coordination of care throughout the surgical continuum. Provides comprehensive education and support to enhance patient understanding, satisfaction, and clinical outcomes.

Key Responsibilities

• Build and maintain strong relationships with referring physicians and specialty providers.
• Guide patients through the continuum of care, from referral and consultation to surgery and recovery.
• Provide individualized pre‑operative and post‑operative education tailored to patient needs.
• Improve patient understanding of diagnoses, treatment options, and care plans.
• Promote informed patient decision‑making.
• Ensure timely scheduling of patient appointments, procedures, and follow‑ups. Regular, proactive review of provider schedules to ensure appropriate referrals and scheduling.
• Facilitate referrals to consultants and ancillary services as directed by the physician.
• Coordinate additional care, including diagnostic testing, procedures, hospital admissions, and physician orders.
• Initiate and manage pre‑authorizations for specialty medications, procedures, and treatments.
• Manage and monitor patients through the electronic health record (EHR), ensuring adherence to active treatment plans.
• Serve as a liaison between patients, providers, hospitals, and multidisciplinary teams.
• Identify and mitigate barriers to care to improve access and continuity.
• Support program growth by strengthening relationships with referring practices and enhancing the patient experience.

Direct Patient Care & Clinical Support - 30%

Provides hands‑on clinical assistance to ensure safe, efficient, and high‑quality patient care.

Key Responsibilities

• Triage patients, obtain vital signs, and act as a chaperone for providers.
• Assist with office‑based surgical and diagnostic procedures.
• Oversee and appropriately route triage calls and EHR messages.
• Deliver individualized nursing care to meet patient needs.

Documentation, Compliance & Health Information Management - 10%

Ensures accurate documentation and adherence to regulatory and organizational standards.

Key Responsibilities

• Document all patient encounters within the EHR.
• Record pertinent patient information and complete workflows to meet meaningful use and advancing care information objectives.
• Complete patient forms, including FMLA, school forms, and work notes.
• Ensure compliance with departmental, institutional, and regulatory policies and procedures.

Operational Support & Organizational Responsibilities - 10%

Supports the efficient operation of the Minimally Invasive Gynecologic Surgery (MIGS) program and clinical environment. Demonstrates professionalism and alignment with institutional goals and expectations.

Key Responsibilities

• Promote the philosophy, goals, and objectives of the institution, department, and program.
• Provide education and serve as a clinical resource for staff.
• Maintain required attendance to meet performance expectations.
• Assist with tracking metrics, statistics, and program performance indicators.
• Support workflow optimization and operational efficiency.
• Complete other duties as assigned.

Minimum Qualifications And Skills

EDUCATION

RN required. Completion of BLS or other approved CPR courses required with recertification every 2 years.

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions and treatment protocols. Ability to write routine reports and correspondence required. Ability to effectively present information in one‑on‑one situations to those coordinating care for a patient.

MATHEMATICAL SKILLS

Ability to calculate figures and amounts required for medication administration required.
