Taho e Forest Health System in Truckee, CA is seeking a Registered Nurse Care Coordinator. The role focuses on coordinating care for medically complex patients, developing patient-centered plans, and collaborating with the care team to improve outcomes.

The ideal candidate has CA RN licensure, an accredited nursing program, and strong communication, organizational, and education skills. On-site duties, with a full-time schedule and ERM/comms support provided.

Formación

• Associate's Degree in Nursing from an NLN/CCNE accredited program or equivalent.
• Current California RN license.
• BLS certification required upon hire or within 3 months.

Responsabilidades

• Perform nursing process: assessment, planning, implementation and evaluation.
• Act as patient care coordinator and advocate within the care team.
• Facilitate patient education and self-management support.
• Coordinate home monitoring and medication management per plan.
• Collaborate with internal/external providers and community resources.
• Engage in data collection, outcome reporting, and audits.
