Formación

• RN licensure in good standing is required.
• ADN or equivalent nursing degree with clinical experience.
• Experience coordinating care for medically complex or underserved patients.

Responsabilidades

• Act as patient advocate and navigator to help access services.
• Educate and counsel patients and families on results and care plans.
• Coordinate referrals for social determinants of health and specialists.
• Participate in care team meetings and hospital rounds as needed.
• Lead health education and group visits, and follow up after hospital events.
• Maintain communication across the care team and with patients’ families.
