KenCrest is seeking a Nurse Care Coordinator (RN) in Stratford, CT. This full-time hybrid role blends in-field visits, in-office work, and remote collaboration to coordinate comprehensive care for individuals with intellectual and developmental disabilities.

You will partner with Program Managers, complete health assessments, document nursing notes in the EMR, and ensure timely treatments, follow-up, and education for direct care staff across assigned homes.

Qualifikationen

• Valid Registered Nurse (RN) license in the State of Connecticut.
• DDS nursing orientation upon hire or 6 months after hire.
• Certified in Health care provider CPR (BLS).
• 3 years' nursing experience working with people with developmental disabilities, preferred.
• Basic computer knowledge (Outlook, Microsoft Office).
• Valid Driver's License.
• Clear criminal record and other background checks as required by state.

Aufgaben

• Coordinate and manage care for an assigned caseload in a residential setting.
• Complete required Health Assessments and document FOCUS nursing notes in EMR.
• Manage hospitalizations and biweekly home visits.
• Provide education and training to direct care providers to uphold continuity of care.
• Collaborate with Program Managers to coordinate treatment, appointments, and follow-up.
