Job summary

The South Bend Clinic is seeking an RN Case Manager for Care Ally at the Eddy Street Campus in South Bend, IN. You will work within a physician-led team to plan and coordinate patient care, educate patients, and drive care management activities across disciplines.

You will collaborate with primary care providers, review care plans, and support transitions of care while adhering to HIPAA and regulatory requirements. Local travel may be required.

Qualifications

• Minimum of an unencumbered license as a Registered Nurse in Indiana.
• Experience in case management or care management; CCM preferred.
• Experience with Medicare/Medicaid eligibility and benefits.
• Experience with multiple EHR/EMR systems.
• Ability to multitask and work collaboratively with care teams.
• Geriatric care, family medicine or long-term care background preferred.
• Strong communication and organizational skills; HIPAA compliant.

Responsibilities

• Assumes an active role in planning and implementing educational needs of patients with the care team.
• Coordinates clinical care plans with PCP and team members; communicates plan to patient/family.
• Review services and enroll patients in the CCM program; implement electronic care plans.
• Determine health and educational needs from calls and chart reviews; maintain monthly contact.
• Collaborate with providers to facilitate physical, behavioral, and social services.
• Manage care plans, make changes as needed, and address complex clinical situations efficiently.
• Identify patients for social worker intervention; support transitions of care at home.
• Communicate utilization, LOS, and post-acute needs with physicians and nursing staff.
