Job summary

Ochsner Health is seeking a registered nurse to provide continuity of care across care settings. You will develop relationships with patients and their multidisciplinary teams to facilitate follow-up, coordinate care plans, and advocate for patient-centered care.

Requires 2 years of nursing experience, a valid RN license in the state of practice, and BLS from the American Heart Association. Strong communication and data-entry skills are essential.

Qualifications

• Required - Registered Nurse Diploma.
• Preferred - Associate's or Bachelor's degree in Nursing.

Responsibilities

• Ensure continuity of care across settings with patient and multidisciplinary team.
• Manage high-risk, complex patient care to minimize readmission.
• Coordinate care plans with physicians and the care team; advocate for the patient.
• Develop, execute and reinforce post-discharge care plans.
• Identify barriers to care and drive process improvements for patient navigation.
