Job summary

Qualifications

• Proficiency in using computers, software, and web-based applications.
• Effective verbal and written communication to diverse staff and patients.
• Excellent conflict resolution skills.
• Ability to work a flexible work schedule (e.g. 24/7, weekends, holidays, on call) and travel between facilities.
• 2 years of nursing experience required.

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops relationships with patients and the multidisciplinary team to coordinate care.
• Manages high‑risk, complex patient care to minimize readmission.
• Coordinates care plan with physicians and team and communicates with the patient advocate.
• Prepares and reinforces post-discharge care plan.
