Qualifications

• Use technology to manage populations and perform comprehensive clinical assessments.
• Inform patients and families about care coordination and work with the healthcare team.
• Reassess patient progress and update care plans as needed.
• Close care gaps around specialty disease and surgical episodes.
• Serve as primary patient contact and advocate for patients and families.
• Assist in transitions of care and identify barriers to receiving care.
• Partner with care coordinator teams to enhance patient care.
• Educate patients and families about managing specialty or surgical conditions.

About This Role

What You'll Do

• Identify patients with ongoing care coordination needs and outline involvement required by the specialty care team.
• Conduct targeted outreach to high-risk patients to ensure timely care delivery.
• Utilize technological tools to manage populations and conduct comprehensive clinical assessments.
• Inform patients and families about care coordination and collaborate with the healthcare team.

Requirements

Education

• Graduate from an accredited school of Professional Nursing
• Bachelor's of Science in Nursing (BSN) preferred

Experience

• 3-5 years of nursing experience

Licenses & Certifications

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification required
