Qualifications

• RN with BSN preferred.
• Minimum of two years of healthcare experience; quality, care coordination and population health exposure valued.
• Current New Mexico Nursing License or eligible to obtain licensure.

Responsibilities

• Provide ambulatory care coordination to support high-quality patient care across the continuum.
• Coordinate patient records across EMR systems and assemble a single patient file; interpret orders with strong critical thinking.
• Collaborate with physicians, administrators, managers, care coordinators and patients to ensure integrated care.
• Develop relationships with SNFs, rehab, LTAC, home health, hospice, palliative care and other community resources.
• Work with interdisciplinary teams to ensure appropriate referrals, follow-up and optimal patient outcomes.
