Job summary

Southwest Care Center is seeking a full-time RN Care Coordinator to support our Santa Fe clinics while residing in Albuquerque or Santa Fe, NM. You will identify high-risk patients, develop interdisciplinary care plans, and educate patients and families about the care process.

You’ll coordinate appointments, tests, and resources, document in the EMR, and help improve clinical practices within our quality programs.

Qualifications

• Associates degree in nursing required.
• Active RN license in New Mexico or compact state.
• Minimum 1 year of RN experience in ambulatory/primary care.
• Bachelor's degree preferred.

Responsibilities

• Identify high risk patients and develop interdisciplinary care plans.
• Educate patients and families about the care coordination process.
• Coordinate care information with other providers and tests.
• Document in the EMR timely and accurately.
• Evaluate adherence to care plans and modify as needed.
• Support quality improvement and participate in team meetings.
