Mount Carmel Health System in Ohio invites applications for an RN Care Navigator. Working within the Ohio Nurse Practice Act, you will coordinate and deliver patient care using the Nursing Process in a team-oriented environment.

Qualifications

• Graduate of a school of nursing.
• Current, unrestricted RN license to practice in Ohio.
• Minimum of two years experience in an ambulatory setting preferred.
• One year Case / Care Management experience preferred.

Position Purpose

RN Care Navigator, within the scope of the Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person’s physical, emotional, social and spiritual needs. The Registered Nurse Care Navigator, functioning in the spirit of teamwork, is accountable for the coordination and delivery of patient care utilizing the Nursing Process within the standards, policies, procedures and guidelines of the Organization.

Hours of office: Monday - Friday 8:00am - 4:30pm

Minimum Qualifications

• Education: Graduate of a school of nursing. Associate's Degree required Bachelor's preferred.
• Licensure / Certification: Registered Nurse; current, unrestricted license to practice in the State of Ohio.
• Experience: Minimum of two years experience in an ambulatory setting preferred.
• One year Case / Care Management experience preferred.
