Mount Carmel Health System in Ohio is seeking an RN Care Navigator to coordinate patient care within the Ohio Nurse Practice Act. You will provide compassionate, team-based care across primary care settings in Columbus and surrounding locations.
Responsibilities include Medicare wellness visits, outreach to patients, and seamless communication with providers using EPIC. Ohio RN license required; two years ambulatory experience preferred; bachelor’s degree encouraged.
RN Care Navigator, within the scope of the Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person’s physical, emotional, social and spiritual needs. The Registered Nurse Care Navigator, functioning in the spirit of teamwork, is accountable for the coordination and delivery of patient care utilizing the Nursing Process within the standards, policies, procedures and guidelines of the Organization.
