Job summary

Mount Carmel Health System is seeking an RN Care Navigator to coordinate patient care in Primary Care settings in Grove City, OH. In this role, you will apply the Nursing Process within organizational standards and work as part of a team to meet physical, emotional, social, and spiritual needs.

The position requires Ohio RN licensure, an Associate's degree (Bachelor's preferred), and ambulatory care experience, with EPIC experience a plus; relocation assistance may be offered.

Qualifications

• Education: Graduate of a school of nursing; Associate's Degree required, Bachelor's preferred.
• Licensure/Certification: Registered Nurse with current Ohio license.
• Experience: Minimum of two years in ambulatory setting; one year case/care management experience preferred.

Position Purpose

RN Care Navigator, within the scope of the Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person’s physical, emotional, social and spiritual needs. The Registered Nurse Care Navigator, functioning in the spirit of teamwork, is accountable for the coordination and delivery of patient care utilizing the Nursing Process within the standards, policies, procedures and guidelines of the Organization.

Hours of office: Monday - Friday 8:00am - 4:30pm

Minimum Qualifications

• Education: Graduate of a school of nursing. Associate's Degree required Bachelor's preferred.
• Licensure / Certification: Registered Nurse; current, unrestricted license to practice in the State of Ohio.
• Experience: Minimum of two years experience in an ambulatory setting preferred.
• One year Case / Care Management experience preferred.
