Job summary

Piedmont Augusta Hospital is seeking a Care Manager (RN) to coordinate care and progression of care across the patient journey, with a focus on proactive discharge planning to ensure timely and safe discharges from the case load.

You will collaborate with inpatient teams, discharge planners, and care management colleagues to optimize patient outcomes. Nursing licensure in Georgia is required, with preferred experience in care management and appropriate certifications.

Qualifications

• Graduate of Nursing Program Required.
• Master’s degree Preferred.
• 2 years of experience in an acute or post-acute setting Required.
• Experience using InterQual or Milliman Preferred.
• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required.
• Certification in Case or Care Management Preferred.
