BoldAge PACE is seeking a Registered Nurse Care Manager to assess care needs, provide nursing interventions, and evaluate outcomes across clinic, center, and participant homes. You will collaborate with the interdisciplinary team to develop plans of care and delegate tasks to aides per patient needs.

You will participate in 24/7 on-call triage, coordinate home care services, monitor acute and chronic needs, and ensure timely follow-ups with providers.

Qualificações

• Bachelor of Science in Nursing Degree preferred.
• State RN License required
• BLS required (have within 90 days of employment).
• 1 year of experience working with a frail or elderly population preferred.
• Experience in home care, long-term care and /or managed care preferred.
• 1 year experience providing care as an RN required.

Responsabilidades

• Provide high quality clinical care as a member of the IDT.
• Provide nursing care in clinic, center, contracted facilities, and participants’ homes.
• Participate in 24/7 on-call triage for participant needs.
• Develop and implement home care plans with the IDT.
• Coordinate care, ensure timely follow-ups and physician communication.
• Reconcile MARs and monitor medication adherence.
• Educate participants and families on self-care and safety.
