BoldAge PACE is seeking an RN Care Manager (Exempt) in Lakehurst, NJ to assess participant needs, deliver nursing interventions, and evaluate care outcomes across clinic, center, and home settings. You will collaborate with the interdisciplinary team to develop and adjust care plans and ensure timely provider follow-up.

The role requires a BSN (preferred), NJ RN license, and BLS certification, with at least one year of RN experience in elder care.

Qualifications

• BSN preferred; RN license required.
• NJ RN license required and in good standing.
• BLS certification required (within 90 days of employment).
• 1 year of experience with frail/elderly population preferred; training provided if not.
• Experience in home care, long-term care, or managed care preferred.
• 1 year of RN care experience required.

Responsabilités

• Provide high quality clinical nursing care as a member of the IDT.
• Coordinate care across clinic, center, home, and contracted facilities.
• Participate in 24/7 on-call triage for participant needs.
• Develop and adjust plans of care with IDT and participants' families.
• Ensure timely follow-up with providers and obtain necessary documentation.
• Supervise CNAs and Home Care Aides as needed.
