Careforth, Inc. is seeking an RN Care Manager to deliver personalized care and support to caregivers and their loved ones. The role offers a flexible schedule with no night shifts or weekends, and no on-call duties.

You will assess needs, create individualized care plans, conduct home visits, and ensure quality consumer care. The position emphasizes travel within the territory, clinical coaching via our app, and collaboration with the care management team to reduce caregiver burnout while

Qualifications

• Must hold an active RN license in the state of employment.
• Minimum two years of recent direct care experience with elders or adults with disabilities.
• Ability to travel within the assigned geographic area; valid driver’s license and auto insurance required.
• Experience in case management, care planning and assessment for eldercare and complex medical needs.
• Proficiency with common software and web-based applications (MS Office, case management systems).

Responsibilities

• Conduct home visits in the assigned territory to assess home safety and health status and provide clinical coaching.
• Follow established care plans to coach caregivers via the app, Referring resources as needed.
• Develop trusting relationships to reduce caregiver burnout.
• Document and review records for accuracy and regulatory compliance.
• Support additional projects and duties as assigned.
