Job summary

Careforth, Inc. is seeking an RN Care Manager to provide personalized care to caregivers and their loved ones across Rhode Island with a flexible schedule and no night shifts or weekends.

You will assess needs, create care plans, conduct home visits, and ensure excellent consumer care, while traveling within the state as needed. This role emphasizes professional compassion, patient-centered coaching via the Careforth app, and collaboration with a national network.

Qualifications

• Active RN license in the state of employment.
• Minimum two years of direct care experience with elders or people with disabilities.
• Ability to travel within the assigned geographic area; valid driver's license and auto insurance.
• Experience in case management, care planning, and assessments in a healthcare setting.

Responsibilities

• Conduct home visits to assess safety and health status and coach caregivers.
• Provide ongoing coaching via app per care plans and refer to resources.
• Develop trusting relationships to reduce caregiver burnout.
• Document and review records for accuracy and compliance.
