Schedule: Full Time

Shift : Monday-Friday 8AM-5PM

Location: Remote. Will meet in Maitland, FL for in-person meetings and must be located within 50 miles.

The role you’ll contribute:

The Nurse Health Advisor coordinates care, educates clients, and provides interventions within the scope of case management practice that have direct influence on clinical and financial outcomes. It is a dynamic and systemic collaborative approach to providing and coordinating health care services to a defined population across the continuum. Responsibilities include identifying high-risk clients, assessing healthcare needs, collaborating with healthcare providers, and developing care plans with accurate documentation. The role involves facilitating options and services to meet individuals' healthcare needs, reducing fragmentation and duplication of care, and improving quality and cost-effectiveness. Active participation in outstanding customer service and maintaining respectful relationships are essential.

• Identify high-risk clients, monitor, assess, and evaluate health care and psychosocial needs, and coordinate care between providers.
• Develop and implement care plans with immediate and long-term goals, document interventions, and activities aligned with care management objectives.
• Follow case management standards and evidence-based clinical guidelines, incorporating whole person care with the CREATION Health model to promote quality and cost-effective outcomes that enhance physical, psychosocial, and vocational health.
• Assess clients’ activation levels, literacy, and self-management skills, referring to appropriate team members or community resources as needed, and integrating these into the care plan.
• Facilitate advanced care planning and collaborate with behavioral health services to prevent unnecessary hospitalizations or readmissions.
• Educate clients and families on diagnoses, medications, nutrition, and promote self-management.
• Monitor and document responses to interventions and client progress, using motivational interviewing to encourage positive behavior change and achieve goals.
• Work with the interdisciplinary team to develop person-centered care plans aimed at preventing avoidable hospitalizations and emergency visits.

Qualifications:

• Graduate of an accredited nursing school
• 5+ years of clinical experience as a registered nurse
• Current registration with the Florida State Board of Nursing
• Valid Florida RN license

Preferred qualifications:

• Bachelor's or higher degree in Science or related field
• Experience with telecommuting, activity tracking, call centers, and related metrics
• Minimum of 5 years in clinical nursing areas such as case management, quality assurance, utilization management, insurance/managed care, hospice/home health, ED/ICU, or geriatrics
• Experience with virtual meetings using WebEx, Skype, FaceTime, Go-To-Meeting, Zoom, or similar platforms for at least 1 year
