We have an exciting opportunity for an experienced Registered Nurse (RN) Care Manager to join our growing Population Health team! The RN Care Manager will work with patients whose chronic conditions are poorly managed and that impact their quality of life, symptom management, and utilization of services. The RN Care Manager connects with patients via telehealth, phone, home, or practice visits, supporting them in managing complex medical needs such as COPD, HF, CKD, and HTN. As a trusted member of the healthcare team, the RN Care Manager works with patients to identify goals that improve quality of life, patient experience, clinical outcomes, and reduce avoidable ED/hospital utilization.

The RN Care Manager reviews patient status, provides ongoing education on condition self-management, identifies Social Drivers of Health (SDOH) related barriers, places community resource referrals, and coordinates services. This position is full-time, remote with occasional on-site practice and home visits.

Our Registered Nurse Care Manager embodies our organizational values of patient-centered quality care, teamwork, and mutual respect. We are seeking a Registered Nurse who will provide care Monday - Friday, during day shifts, 40 hours a week. The ideal candidate will hold an active license and a BSN degree (though not required).

Summary:

The Registered Nurse - Care Manager is responsible for managing high-risk patients to promote effective education, self-management, and timely healthcare delivery to achieve optimal quality and financial outcomes. The role also involves ensuring safe patient transfers across the healthcare continuum, acting as a bridge between professional staff and patients or families, especially during care transitions.

Required Minimum Knowledge, Skills, and Abilities (KSAs):

  1. Education: Graduate of an accredited School of Nursing required; BSN preferred.
  2. License/Certifications: Current applicable state license as a Registered Nurse required. BLS certification required or must be obtained within 30 days of start date.
  3. Experience: Three years of direct clinical care experience preferred.
