Job summary

EGA Associates, LLC is seeking a qualified Registered Nurse Care Manager to support a Patient Aligned Care Team at a Primary Care Community Based Outpatient Clinic serving Veterans in Pottsville and surrounding Schuylkill County, PA. The clinic delivers outpatient primary care services in a team-based setting.

Responsibilities include developing care plans, coordinating services, and ensuring timely, evidence-based nursing care.

Qualifications

• Graduate of a nursing program accredited by ACEN or CCNE.
• Current, full, active RN license in PA or a multi-state compact state.
• Current BLS certification or ability to obtain before patient care.
• Ability to complete VA and EGA credentialing, background screening, onboarding requirements.
• Knowledge of care management, care coordination, chronic disease management, population health, and outpatient nursing operations.

Responsibilities

• Provide professional nursing services per licensure, certification, and standards of practice.
• Develop and manage care management plans in collaboration with PACT staff.
• Participate in secure messaging, virtual care, view-alert management, and visits.
• Identify patient needs and discuss RN recommendations with the Primary Care Provider.
• Engage PACT staff and enter consultation requests as needed.
• Delegate activities to qualified clinical/administrative associates while remaining accountable.
• Enter orders for tests under RN care-management protocols.
• Provide same-day access for face-to-face and telephone care.
• Use evidence-based guidelines to promote patient engagement and self-care.
• Participate in quality and performance-improvement activities.
