Job summary

EGA Associates is seeking a Registered Nurse Care Manager to support a Patient Aligned Care Team at a Primary Care Community Based Outpatient Clinic serving Veterans primarily residing in and around Idabel and McCurtain County, Oklahoma. The clinic provides primary care services in an outpatient clinical environment.

The RN will work 8:00 AM to 4:30 PM, 40 hours per week, with travel negligible. Candidates must be licensed in Oklahoma, familiar with VA processes, and able to operate EHRs and

Qualifications

• RN license in OK or compact jurisdiction.
• Experience in care management and coordination.
• Knowledge of outpatient nursing operations.
• Ability to use EHRs and telehealth platforms.

Responsibilities

• Provide professional nursing services to assigned and unassigned patients.
• Develop, oversee, and manage care management plans and care coordination.
• Participate in secure messaging, virtual care, and face-to-face visits.
• Identify patient needs for discipline-specific team members and discuss nursing recommendations.
