Job summary

Community Health Network is seeking a Care Manager RN for Post-Acute Care IHCI to lead patient transitions across SNFs within the ACO. You will perform comprehensive assessments, develop care plans, coordinate services and monitor outcomes to improve quality and cost-effectiveness.

In this role, you will collaborate with nursing staff, physicians and facility partners to ensure seamless continuity of care, reduce readmissions and advocate for patients and families throughout the care journey.

Qualifications

• 2 year / associate degree in nursing (required).
• Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) (required).
• Preferred Certifications: CCM, ACM, or equivalent certification.
• 3+ years: nursing experience, preferably in Population Health management, Case Management, or Post-Acute Care. (required)

Responsibilities

• Support the healthcare team towards a smooth transition from one level of care to another in support of the patient/family.
• Provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs.
• Manage post-acute services for patients across skilled nursing facilities (SNFs) within the ACO; monitor length of stay and coordinate care plans with facility staff.
• Collaborate closely with facility staff to address patient needs.
