Job summary

Qualifications

• Bachelor of Science in Nursing from an accredited School of Nursing.
• GA RN license or valid multi-state (NLC/eNLC) license.
• Three (3) years of clinical care experience.

Responsibilities

• Oversee care management and coordination for high-risk patients.
• Promote population health through education, self-management support, goal setting, and timely health care delivery.
• Develop and monitor care management processes for primary clinical teams.
• Collaborate with practice personnel to best serve the needs of identified patient panels.
